CONNECARE EURECAT

Integrated and personalized management system based on participative, predictive and preventive medicine to provide a better quality of life to chronic disease patients. Connecare project develops a technological solution that allows a more efficient monitoring of the patient and a reduction of sanitary costs.

The solution developed by Connecare is based on P4 Medicine (Participative, Personalized, Predictive, and Preventive) involving a paradigm shift that will promote managing preventive medicine in patients’ homes. Aside from this, Connecare will also offer a technological solution developed through a combination of different types of technology resources to improve patient monitoring.

The Connecare solution will therefore significantly reduce health costs across Europe by guaranteeing a much more efficient monitoring of chronic diseases that ultimately will lead to better quality of life for patients.

The Connecare consortium is made up of nine partners from six different countries: Eurecat, the August Pi i Sunyer Biomedical Research Institute, the Lleida Biomedical Research Institute’s Dr. Pifarré Foundation (Spain), the University Medical Center of Groningen (the Netherlands), Advanced Digital Innovation (UK), eWave, Assuta Medical Centers (Israel), the Technical University of Munich (Germany), and the University of Modena and Reggio Emilia (Italy).
